Assistance to Access Community, Social and Recreational Activities supports participants to engage in their community and take part in activities that interest them. This may include attending social events, joining groups, participating in hobbies, or accessing local facilities.
Supports are tailored to build confidence, social skills, and independence, while reducing barriers to participation. The focus is on helping participants feel included, develop meaningful connections, and enjoy a more active and fulfilling lifestyle.
School leaver employment supports help participants move from school to work and are available in the final years of school and directly after leaving school. Providers who deliver school leaver employment supports help young people prepare, look for and gain employment.
Exploring what work would mean for you, building essential foundation skills for work; managing complex barriers to obtaining and sustaining employment; Supports to transition from an Australian Disability Enterprise (ADE) to open employment, developing a career plan and Engaging in capacity building supports that are likely to lead to successful engagement in a Disability Employment Service (DES).
Life Transition Planning supports participants through key changes and stages in their lives, such as moving into independent living, starting employment, or adjusting to new routines. This support includes mentoring, peer support, and individual skill development to build confidence and resilience during these transitions.
The focus is on developing practical skills, strengthening decision-making, and providing guidance to navigate change successfully. Supports are tailored to the individual, helping them feel prepared, supported, and empowered to move forward and achieve their goals.
Skill Development and Training supports are designed to build practical, everyday skills that increase independence and confidence. This can include areas such as public transport training, budgeting and money management, and other life skills tailored to the participant’s goals.
Supports are delivered in a hands-on, individualised way, helping participants develop the knowledge and confidence to navigate their community, manage finances, and make informed decisions in daily life. The focus is on empowering participants to live more independently and achieve greater control over their routines and future.
NDIS Support Coordination is a capacity-building support designed to help participants understand and use their NDIS plan effectively. A Support Coordinator works alongside the participant to connect them with suitable services, build their confidence in managing supports, and ensure their plan is implemented in a way that aligns with their goals.
This includes assisting with choosing providers, coordinating multiple supports, navigating challenges, and building the participant’s ability to manage their own supports over time. The focus is on increasing independence, strengthening decision-making, and ensuring supports are working well together to achieve meaningful outcomes.
NDIS Counselling is a therapeutic support that helps participants build emotional wellbeing, resilience, and coping strategies in relation to their disability and life experiences. It provides a safe, supportive space to explore challenges such as anxiety, stress, behavioural concerns, relationships, and life transitions.
Sessions are tailored to the individual, focusing on practical strategies, emotional regulation, and building insight to support everyday functioning. The goal is to strengthen confidence, improve quality of life, and support participants to achieve their personal goals in a way that is meaningful to them.
